The three-week rule, the signs that separate a harmless ulcer from a dangerous one, and what to do next.

The rule that settles it: three weeks

Almost everyone gets mouth ulcers — from bites, braces, sharp teeth, spicy food, stress, or no reason at all. Ordinary ulcers hurt, look pale yellow or white with a red rim, and heal within ten to fourteen days. The rule that matters is simple: any ulcer, patch or lump in the mouth that has not healed in three weeks needs a doctor to look at it. Not another gel. Not another month. An examination.

What makes an ulcer suspicious

Who should be extra careful

Anyone who uses tobacco in any form — gutkha, khaini, pan masala with tobacco, cigarettes, bidis — carries a many-fold higher risk of oral cancer, and so does anyone who chews areca nut (supari) alone. For users, the three-week rule tightens to: get any persistent mouth change examined, and get an annual mouth check even with no symptoms. It takes minutes and it is free at our screening camps.

What happens when you get it checked

A specialist examines the ulcer and the neck. If it looks harmless, you leave reassured — most do. If it looks suspicious, a small biopsy under local anaesthesia settles the question in days. Caught early, oral cancer is one of the more curable cancers, often with a smaller operation and excellent recovery of speech and eating. The entire difference between the easy road and the hard one is usually the month someone waited.

Common questions

My ulcer keeps coming back in different places. Is that cancer?
Recurrent ulcers that appear, heal fully within two weeks, and reappear elsewhere are typically aphthous ulcers — troublesome but not cancer. The concerning pattern is one ulcer, in one place, that never fully heals.
Can a mouth ulcer be cancer even if it does not hurt?
Yes — early oral cancers are frequently painless. Waiting for pain is the most common and most costly mistake. Persistence beyond three weeks is the sign that matters.
I chew gutkha. How often should I get my mouth checked?
